Once I receive your inquiry, I will review and follow up with you on availability to schedule a free 15 minute consultation phone call.

Step 2

The consultation phone call is a chance for us to get to know each other. During this call, we make sure that we are a good fit for each other and discuss any concerns.

Once we move forward, I will provide you with a link to schedule your intake appointment, complete necessary documents, and verify your insurance.

You are required to complete the preliminary paperwork and questionnaires 48 hours prior to your appointment to avoid cancellation/rescheduling.
